Pain and discomfort related to the spine are extremely common, to the extent that back and neck pain may be considered part of the human condition. In most instances, interventions are not necessary. However, surgery can be required in some situations, and may be considered as a valuable option in others. In order to enable accurate patient counselling and to help in fine-tuning indications for surgery (for example, after unsatisfactory relief from conservative pain management), physiotherapists and rehabilitation specialists should be aware of surgery rationales and techniques. In most cases, surgery does not stand on its own, and postoperative rehabilitation will benefit from a certain degree of surgical knowledge by the physiotherapist and rehabilitation specialist. Additionally, it is paramount that all spinal caregivers speak the same vocabulary and issue the same messages to the patient. This little book explains surgical reasoning and surgical techniques to non-surgeons. It aims at crossing the bridge between surgical and rehabilitation paradigms to the benefit of patients. Belonging to this very old University also requires a duty of memory, fulfilled by this book that recalls the importance of multiculturalism at the beginning of this University, the will of religious and/or political authorities over the centuries to support the development of this institution, and the importance of focusing on Human beings in the medical approach, according to the Hippocratic principles. Celebrating 800 years is a reminder of these principles in a modern world where humanist values are often undermined by the rapid pace of scientific progress.
Nous devons beaucoup à Jean Marc Bourgery (1797-1849) et à son Traité complet de l'anatomie de l'homme, qui marque un jalon essentiel de l'histoire de la médecine, mais demeure surtout l'un des traités anatomiques illustrés les plus beaux et les plus complets jamais publiés. En 1830, après avoir obtenu son doctorat de médecine - avec trois ans d'avance! -, Bourgery commence à travailler sur son magnifique traité en collaboration avec l'illustrateur Nicolas Henri Jacob (1782-1871). Les premiers volumes sont publiés dès l'année suivante, mais l'achèvement du traité nécessitera un travail assidu de près de vingt ans. Bourgery a vécu juste assez longtemps pour achever son oeuvre de passionné, mais l'intégralité des huit volumes du traité n'a été publiée que cinq ans après sa mort.
Les quatre parties du traité de Bourgery englobent l'anatomie descriptive, l'anatomie chirurgicale et ses techniques (explorant en détail l'ensemble des plus grandes opérations réalisées pendant la première moitié du XIXe siècle), l'anatomie et l'embryologie générales, ainsi que l'anatomie microscopique. Les spectaculaires lithographies grandeur nature mises en couleur à la main sont remarquables par leur clarté et leur qualité chromatique et esthétique, et reflètent un mélange d'observation directe en laboratoire et de recherche artistique; la qualité de ces planches demeure inégalée dans le domaine de l'illustration anatomique.

Appropriate use criteria in echocardiography are essential tools for aligning indications of echocardiography with the best clinical standards, improving clinical outcome, restraining abuse and preserving healthcare resources. Assessment of 501 inpatients who received transthoracic cardiac echo was conducted according to the 2011 appropriate use criteria report. Indications were classified as appropriate, uncertain or inappropriate, and patients not matching any of the above divisions were grouped in the non-fitting category. Of the 501 eligible patients, 374 (74.65%) were in the appropriate group, 85 (16.96%) in the inappropriate group, 20 (3.99%) in the uncertain group and 22 (4.39%) in the non-fitting category. Interpretation and analysis of the obtained results are presented, along with results of many comparable studies; moreover, a quality improvement project was set up accordingly. Appropriate use criteria are useful to assess local practice, to preserve healthcare resources, and to improve clinical outcome.
Transplantation tolerance may be defined as normal graft survival sans immunosuppression, maintaining intact immunity. Tolerance induction (TI) strategies include induction of chimerism and/or clonal deletion. Donor specific transfusion (DST) and stem cells (SCs) have come-up as TI. In this parallel 2-armed open labeled study under non-myeloablative conditioning with DST as first arm and SCs as second arm, without using conventional immunosuppression. Monitoring included SCr (mg/dL), DSA and MLR. Protocol biopsies were planned after 100 days and yearly in willing patients. In event of rejection/DSA/MLR, rescue immunosuppression was planned. Patient survival for the first 2 years was 100% in both arms; at 5 years post-transplant 80% with SC and 90% with DST. Death-censored graft survival for 1st year was 100 % in both arms, and at 5 years, 80% with SC and 70% with DST. Mean SCr was 1.52 with SC and 1.97 with DST. Five patients from SC-arm and 3 from DST-arm had successful TI. MLR was negative in both groups. DSA appeared in 2 patients of SC without graft dysfunction and 4 of DST causing graft dysfunction. Thus SCs facilitate TI in LDRT under non-myeloablative conditioning.
Endoscopic ultrasound-guided biliary drainage (EUS-BD) is being increasingly used as an alternate therapeutic modality to percutaneous transhepatic biliary drainage (PTBD) for biliary obstruction in patients who fail ERCP. We conducted a systematic review and meta-analysis to compare the efficacy and safety of these 2 procedures. We searched several databases from inception to September 4, 2016, to identify comparative studies evaluating the efficacy and safety of EUS-BD and PTBD. Primary outcomes of interest were the differences in technical success and postprocedure adverse events. Secondary outcomes of interest included clinical success, rate of reintervention, length of stay in hospital, and cost comparison for these 2 procedures. Odds ratios (OR) and standard mean difference were calculated for categorical and continuous variables respectively. These were analyzed using random effects model of meta-analysis.
Les Heligmonellidae représentent la famille la plus riche en espèces parmi les Heligmosomoidea. Ces derniers font partie de l'une des trois super-familles des Trichostrongyloidea, Nématodes parasites du tube digestif de la plupart des Vertébrés terrestres. La famille comprend plus de 360 espèces, principalement parasites de Rongeurs et distribuées dans le monde entier. Leur cycle biologique étant monoxène et leurs hôtes relativement faciles à élever au laboratoire, ces parasites peuvent servir de modèles pour les chercheurs, leur permettant d'aborder des sujets tels que les relations hôte-parasite, la biologie, la pathogénicité et la phylogénie. Cependant, la très grande diversité du groupe, jointe à une apparente homogénéité morphologique, rend leur identification extrêmement difficile. Cette révision critique présente pour la première fois, une compilation de toutes les données taxonomiques, morphologiques et morphométriques disponibles concernant la famille des Heligmonelllidae. Son but est de fournir aux chercheurs les outils nécessaires pour identifier les taxa connus le plus précisément possible. Quatre des cinq sous-familles reconnues sont traitées ici : les Tricholinstowiinae, les Heligmonellinae, les Pudicinae et les Brevistriatinae.

The third volume of the series of Atlases deals with the use and usefulness of electroencephalography (EEG) in neurology.
While EEG is universally recognized as a first-order investigation method in epilepsy (see Volume 2), and as an important contributor in sleep medicine, practical neurology has tended to neglect the value of this classical and established neurophysiological tool. A rich, extensively commented and analyzed collection of EEG plates is presented here.
The reader will be compelled to remember that EEG is the easiest way to assess parameters like state of vigilance, risk of seizure activity, type and degree of functional impairment, in a very clinical and practical setting. The authors cover many aspects of neurological practices where the EEG may help in diagnosis and treatment: metabolic and other encephalopathies, infectious and inflammatory conditions, vascular disorders. It is particularly useful -and difficult- to distinguish between epileptic phenomena and EEG changes associated with metabolic abnormalities: a careful assessment of the EEG is of paramount practical importance here. Migraine is not always simple and there are many overlaps with other types of neurological diseases: the EEG may play a major part in helping the clinician in doubtful cases. Lastly, even the neurosurgeon may need the EEG to monitor trauma, tumor, bleeding...

Encephalopathy with Electrical Status Epilepticus during Sleep (ESES)» was described almost 50 years ago; however its clinical spectrum and its boundaries, the diagnostic criteria, the pathophysiology and the therapeutic management are still debated. In the last years, clinical data, neurophysiological and imaging investigations, as well as genetic studies have renewed the interest on ESES. In addition, experimental findings from sleep research have opened fascinating perspectives on some possible pathophysiological mechanisms involved in this condition.

Not only humans and other biological creatures, but also cultures and sciences, communities and corporations have a life of their own; they develop and grow; they are successful or fail; they communicate and cooperate; they may be in conflict or in harmony with each other. The term 'bio-ethics' was coined by Fritz Jahr (1895-1953), a Protestant pastor and educator in Halle an der Saale, in 1926 with the intention to call for a conceptual and practical broadening of moral obligations not just to fellow humans but towards all forms of life and symbiotic living-together. His Bioethical Imperative 'Respect every living being in general as an end in itself and treat it, if possible, as such!' did not find much recognition or support in the weltanschauung battles before and during the Nazi times in Germany. We all have learned that the 'birth of bioethics' occurred in 1976 in the United States of America as a 'bilocal birth' in Madison, Wisconsin, and in Washington, DC, associated with the names of Van Rensselar Potter, Andre Helleghers, and Sargent Shriver; and some of us had been involved developing and distributing this model of 'bioethics' as being somewhat identical with medical ethics. It was only at the beginning of this century that Jahr in his integrative and integrating approach was rediscovered as the true «Father of Bioethics».

The objective evaluation of facial aging is a difficult exercise. Many publications have appeared over the past few years that seek to classify facial aging : crow's feet, glabellar wrinkles, ear lobe aging, neck sagging, etc. These publications show the advantages and need for objective evaluation tools for facial aging. These tools make it possible to classify homogeneous patient populations and greater objectivity in the study and evaluation of the "before/after" results of anti-aging treatments.
"Skin Aging Atlas" is the latest contribution to these classifications. It is also the first true topographic atlas of facial aging. In a single booklet, it offers all the criteria for evaluating the aging of Caucasian men's and women's facial skin.
In the first part, this atlas, assembled by R. Bazin, describes the single methodology used for each criterion.
Each classification thus allows a precise grade to be given to a specific region of the face (glabellar wrinkles, crow's feet;, neck sagging, etc.). Thanks to this unique methodology, the classifications can also beused together to evaluate the general aging of the face.

Le dictionnaire de médecine dont Preder poursuit la publication comportera douze volumes. Mis en chantier il y a cinquante ans alors que l'auteur était encore étudiant, il a été tenu à jour en fonction du progrès des diverses spécialités et du développement des nouvelles disciplines comme l'immunologie moléculaire.
Chaque volume est composé de deux parties : la première, imprimée sur papier blanc, correspond à une tranche de 160 pages du dictionnaire lui-même; la seconde, imprimée sur papier de couleur, contient un lexique indexé breton-français des termes médicaux utilisés dans la partie dictionnaire (emprunts, néologismes, mots courants pris dans une acceptation particulière).
